Use and Disclosure of PHI

AAA Pharmacy will use and disclose PHI for the following purposes:

• Payment: To obtain payment for healthcare services provided.
• Healthcare Operations: To conduct quality assessment and improvement activities, training programs, and insurance functions.

6. Safeguards

7. Breach Notification

In the event of a breach of unsecured PHI, AAA Pharmacy will notify affected individuals and the Department of Health and Human Services as required by HIPAA regulations.
